Scheduling and General Information
New patients and second opinions

Call the patient coordinators at (312) 695-0990 to schedule an appointment.

When you call to schedule your second opinion consultation our patient coordinators will guide you through the process of gathering your outside imaging studies, pathology slides and other records.  These materials should arrive at Northwestern 5-7 days before your scheduled appointment so that they can be reviewed and an official report completed prior to your consultation. Northwestern uses a service called eHealth which will help gather these materials from your outside institution.   One of our Lynn Sage Breast Center nurses may also contact you prior to your visit to gather further information.

If you have a newly diagnosed breast cancer and are unable to schedule an appointment in a timely fashion, please email me directly at [email protected].  Clinics are usually quite full but I will  try to expedite an appointment.
ITEMS TO BRING

We need to gather all the pertinant information including: mammograms, ultrasound, MRI, and pathology results. These will help us develop a plan tailored to your specific circumstances.

There will be plenty of time to ask questions, seek second opinions, and make an informed, rational decision that is best for you.

There is a lot of new information provided during the consultation and it is difficult to absorb all of it.  Feel free to record our consultation with your smartphone recorder app or conference call a family member or friend.
FORMS TO COMPLETE

  • Preregistration Packet 
    This packet containing basic medical forms will be emailed to you once you schedule an appointment.  Please complete prior to your visit and bring with you to your consultation.  

  • Outside Film Checklist 
    If you've had imaging and/or a breast biopsy performed at another institution you will be emailed this form once you schedule an appointment.  It is an Itemized list of instructions for obtaining breast imaging studies such as mammograms, ultrasound and pathology slides.  You must submit your mammogram, ultrasound and breast MRI images as well as pathology slides at least one week prior to your visit so that they can be reviewed and confirmed prior to your clinic visit.
